Community Case Management

Community case managers pick up when residential services end, providing clients with assistance in obtaining ongoing services. Clients receive assistance with outpatient mental health and primary care appointments, housing placement and other appropriate services.  Once linkages have been established, the dedicated case manager meets individually with the client where they are to provide continued resources and support.
